Bad tongue posture, underdeveloped hyoid muscle (the muscle that holds the tongue) and most importantly, recessed chin. These are my speculations.

Some people, at whatever weight, have always had an extra little pocket of fat there just as some people have an extra little roundish pocket of belly fat even at their thinnest.
These things only get worse with age and weight fluctuations.
