I've worked with a former assistant on this how and she told me it's mostly true, vetted, while they always encourage people to act out. You can tell specially in this video the fight is real, the humiliation too. There's an old pilot episode of behind the scenes you can watch online and she says it's pretty much accurate to the real thing.

He told me he was at a mall or something and a Springer producer approached him and offered him cash if he'd come on the show and pretend he was some whore's babydaddy. He declined.

A guy I went to high school with went on Sally Jessy Raphael to "confront his childhood bully", also a guy who went to our high school. This was about 10-15 years after we all graduated. The show contacted the bully and told him only that "someone from your past has something they want to say to you." He had no idea who it was or what it was about, but he didn't want to pass up a free trip to NYC.
Sally interviews the first guy, who gives her a big sob story about how the bully made his life so miserable. Then they brought out the bully, and the guy proceeds to tell him how horrible the bully treated him growing up. But he wasn't going to let that hold him back. He had moved on with his life and was pursuing a singing career, blah, blah, blah.
The bully looks at him and said, "Dude, I don't even know who you are."
